Formatting the OJT trainer template table.

Modify the OJT trainer template to:

• Modify the current trainers table to be listed as "Additional trainer"
• Place a box around "Trainees name" and "Initials"
• Create a field/table for "Primary Trainers" name & initials

AC:

• The Trainee's name and Initials area is converted into a table
• The Trainer's Name and Initials/Date table is split into two tables: one for the Primary Trainer and one for the Alternative Trainers
