New KSA Issue Template - Missing required Information?

Expected Behavior

When creating a new KSA we should have to specify the minimum criteria necessary for the JSON

• Workrole/Specialization it applies to
• Proficiency code
• Requirements Source/Owner (or derived)
• Mapping
  • Either mapping to parent(s)
  • or req src/req own

Current Behavior

Template does not ask for these items
