SUFFIX requires dc=us

The static dc value of 'us' in init.ldif does not work with any SUFFIX env var, including the supplied default, unless it begins with 'dc=us'. The dc value at the referenced location should probably be parsed from the supplied SUFFIX, or provided by way of another env var.
